Premium rates are determined according to the HIPIOWA plan you select, your age, your
gender and whether or not you use tobacco.
PLAN E is a Medicare eligible preferred provicer plan for persons under 65 that are
enrolled in Medicare. The plan pays secondary to Medicare and has an annual deductible
of $1000.
PLANS B, C, D, F and G offer five deductibles and are preferred provider plans that require
you to obtain services from a participating network provider in order to receive maximum
benefits
Premium payment cycles. During the application process, you will need to decide upon a
premium payment cycle. HIPIOWA offers several different payment cycles, including monthly
bank draft (premium is automatically deducted from your bank account), quarterly, semi-annually
and annually.
If you choose the monthly bank draft, you will also need to complete the automatic bank
withdrawal authorization form which can be downloaded from this web site.
Use the rate tables to determine your premium rates. First, select a plan and deductible
level and then locate the correct rate table. Next, locate your age group, gender and your
premium rate. The premium rates listed are monthly. If you plan to pay quarterly, multiply
the rate by three; semi-annually multiply by six and annually multiply by twelve.
